Disclaimer
We collect personal medical and lifestyle information to help us assess and make your treatment expectations more comfortable. It is your responsibility under the requirement of the Public Health Act you declare that you are not suffering from nor are a carrier of any communicable disease, including Hepatitis B, and Acquired Immune Deficiency Syndrome. Your practitioner will recommend the appropriate schedule for future treatments or physician referral in order to achieve your skin improvement.
COMPETITION & SURVEY TERMS AND CONDITIONS
Employees of Dermal Essence and their immediate families are ineligible to enter. By entering the Dermal Essence competition’s or survey’s, entrants will be deemed to agree with these terms of entry.
Competitions or surveys can be completed by each registered member once only.
The winner of the prize will be notified via email or phone within 28 days
Our decision is final, and no correspondence will be entered into.
Prizes must be taken as stated and no compensation will be payable if any prize winner is unable to use the prize as stated. Prizes are not exchangeable or redeemable for cash or other goods or services. A prize cannot be transferred to any other person unless agreed to by Dermal Essence.
If the prize (or part of any prize) is unavailable, Dermal Essence, in its discretion, reserves the right to substitute the prize (or that part of the prize) with an alternative prize to the equal value and/or specification, subject to any written directions from a regulatory authority.
These terms and conditions do not exclude or limit the application of any statutory provision where to do so would contravene that statute or cause any part of these terms and conditions to be void.
Eligible entrants consent to Dermal Essence using the eligible entrant’s name, likeness, image and/or voice in the event they are a Winner (including photograph, film and/or recording of the same) in any media for an unlimited period without remuneration for the purpose of promoting this competition (including any outcome) of treatment or product.
If a competition or survey is interfered with in any way or is not capable of being conducted as reasonably anticipated due to any reason beyond the reasonable control of Dermal Essence, the promoter (Dermal Essence) reserves the right, in its sole discretion, to the fullest extent permitted by law (a) to disqualify any entrant; or (b) to modify, suspend, terminate or cancel the promotion, as appropriate.
Any cost associated with accessing the promotion is the entrant’s responsibility. The use of any automated entry software or any other mechanical or electronic means that allows an entrant to enter repeatedly automatically is prohibited and will render all entries submitted by that entrant invalid.
Except for any liability that cannot be excluded by law, the promoter (including its officers, employees and agents) excludes all liability (including negligence or mistake), for any personal injury; or any loss or damage (including loss of opportunity); whether direct, indirect, special or consequential, arising in any way out of the competition/survey including, but not limited to, where arising out of the following: (a) any technical difficulties or equipment malfunction (whether or not under the promoter’s control); (b) any theft, unauthorised access or third party interference; (c) any entry or prize claim that is late, lost, altered, damaged or misdirected (whether or not after their receipt by the promoter) due to any reason beyond the reasonable control of the promoter; (d) any variation in prize value to that stated in these Terms and Conditions; (e) any tax liability incurred by a Winner or entrant; (f) participation in or use of a prize.
Dermal Essence collects personal information in order to conduct the promotion and may, for an indefinite period unless otherwise advised, use the information for promotional, marketing and publicity purposes including sending electronic messages or telephoning the eligible entrant. Eligible entrants should direct any request to access, update or correct information to Dermal Essence.

For each person visiting our site, information is automatically logged about you and how you use the website. For example your geographic location and the pages you view. This information and data is used for internal review purposes and allows us to improve our site and marketing.
We collect information volunteered by people on our site. For example, data submitted via forms.
